Two RNAC positions needed in Mechanicsburg Pennsylvania.

Currently, we are seeking the following team members:
Lead RNAC (RN)— FT M-F
The Lead RNAC is responsible for the timely and accurate completion of the Federal and State assessment too, the Minimum Data Set. Utilizes ADL’s for resident assessments; defines appropriate criteria for determining patient status; determines the level duration, and types of patients’ rehabilitation needs in collaboration with the Rehab Manager. Must be able to work with other departments in care planning and coordination. Demonstrates knowledge in Medicare reimbursement mechanisms. Understands the financial concepts and implications of RUG III and PPS.
Registered Nurse, Licensed in Pennsylvania and 5 years of long term care experience required.
RNAC - FT M-F
Responsible for the completion of the Residents' Minimum Data Set (MDS), care plan development and coordination and the coordination and facilitation of the care plan conferences.
